Student File/Transcripts
Each student has a permanent file in the school
office. This includes an official transcript
of academic records. This file is available
for inspection by the student and/or parents
upon request. A summary of standardized test
scores is also available.
All transfer students or otherwise
new students must have medical and dental records
in the school office before schedules are issued
at the beginning of the school year. Official
academic school records are mailed directly
to the institution to which the student is seeking
admission. No official records will be given
to a student or sent to a student.
Accidents
Accidents must be reported to the school office.
The supervising teacher must fill out an accident
report form. If significant injury has occurred,
parent/guardian will be notified immediately.
Major medical emergencies are directed to St.
Mary of Nazareth Hospital.
Change
of Address/Change of Telephone
Any change of address, home phone number, work
number, etc. should be reported to the school
office and homeroom teacher to ensure that the
school can reach the family in case of an emergency.
Communications
to the Home
Communications between the school and home are
sent with the student in the MONTHLY FAMILY
FOLDER. These communications include important
information regarding the lunch program, the
school calendar and upcoming events. Parents
are asked to inquire with their child regarding
such communications on a daily basis. Parents
are often asked to sign such notices, especially
those regarding academic progress and behavior
issues, to ensure that they actually received
such correspondences. Parents may also send
written requests to the teachers via their student,
but are likewise asked to inquire with their
child regarding the delivery of those items.
Emergencies
School Closing.
When it is necessary to close the school because
of severe weather, St. Stanislaus Kostka School
will make such an announcement via local radio
and/or television stations. At times, the Office
of Catholic Education will make a determination
that all Archdiocesan elementary schools are
closed. In such circumstances, an announcement
will also be made via local media. If the Chicago
Public School System closes because of severe
weather conditions, St. Stanislaus Kostka School
will also be closed.
St. Stanislaus Kostka School
will also conduct regular fire drills and other
emergency drills to prepare students for procedures
in the event of such a disaster. At times this
may necessitate that students leave the building
without coats due to poor weather, since students
must be prepared to act quickly and efficiently
in all circumstances, and may not always have
quick access to their coats for such a drill.
Such drills will be handled efficiently to minimize
exposure to the elements.
Finances
St. Stanislaus Kostka School has a tuition Payment
Plan to maintain our excellent educational program.
Annually, parents are asked to sign a tuition
payment agreement which outlines financial costs
and dates for payment. Parents who are registered
parishioners of St. Stanislaus Kostka Parish
are expected to be weekly contributors to the
parish.
In order to maintain our ability
to meet student and faculty needs, prompt payment
of tuition and fees is necessary. A student’s
classroom privileges will be revoked and promotion
to the next grade level denied if the tuition
and fees are not paid. Failure to make payments
by the specified calendared dates will exclude
the child from school until such payment is
made.
Health
In accordance with the Chicago Board of Health
and the State of Illinois laws of compliance,
each student will submit a written record of
immunization dates which must be in compliance
with state regulations prior to attending classes
at St. Stanislaus Kostka School. Every student
entering preschool, kindergarten, first grade,
and fifth grade must also have a current record
of a doctor’s physical examination on
file. Students will not be permitted to attend
classes without written compliance on file in
the school.
Forms are distributed to the
respective children or to their parents/guardians.
In addition, all students shall be immunized
with two measles inoculations, smallpox, tetanus,
diphtheria, poliomyelitis, and pertussis. A
test for lead poisoning is required for children
who are six years old and under (Illinois School
Code, Section 27-3).
Ordinarily, students will be
excluded from school on October 1st for noncompliance
with this law. County health personnel visit
the school occasionally to check the medical
records of the students. Periodic vision and
hearing tests are given at school by technicians
from the Cook County Department of Public Health.
Parents are notified if results indicate the
need for a more complete examination.
Communicable
Disease. The principal shall notify
the Board of Health when a child is sent home
because of a suspected communicable disease.
In case of absence due to a communicable disease,
a release card from the Board of Health or a
letter from the family physician must be presented
upon the child’s return to school.
Medicine.
Medicine of any type, including aspirin, may
not be given to a student. According to state
law, the school may not dispense medication
to a child. It is the parent’s responsibility
to dispense medication. Exceptions to this policy
are at the discretion of the principal.
Abuse.
St. Stanislaus Kostka School staff are mandated
reporters by law. Therefore, any staff member
who even remotely suspects abuse must, by law,
report it to DCFS authorities.
Smoke-Free
Environment. St.
Stanislaus Kostka School has a smoke-free environment.
No smoking is allowed on the premises. Furthermore,
the school instructs students as to the health
hazards of smoking. Student smoking on campus
is considered a very serious offense and appropriate
discipline measures will be taken.
Insurance
All students must be covered by either a parent/guardian’s
insurance plan or the school plan. Student insurance
is made available to each student at the start
of the school year.
Students who participate in athletic
programs must provide written verification of
a physical examination prior to participation
in any interscholastic contest. Students participating
in athletic programs should show proof of health
insurance coverage.
Noncustodial
Parents
St. Stanislaus Kostka School abides by the provision
of the Buckley Amendment with respect to the
rights of noncustodial parents. In the absence
of a court order to the contrary, a school will
provide the noncustodial parent with access
to the academic records and to other school-related
information regarding the child. If there is
a court order specifying that there is to be
no information given, it is the responsibility
of the custodial parent to provide the school
with an official copy of the court order. Information
will be sent to the noncustodial parent upon
the receipt of a self-addressed, stamped envelope.
Phone
Calls
Students may not receive phone calls during
school hours. Messages must be delivered through
the school office. Outgoing calls are limited
to emergencies and must be made from the main
office with the permission of the principal
or classroom teacher.
Search
and Seizure
St. Stanislaus Kostka School personnel reserve
the right to inspect all school property.
Visitors
All visitors to the school building are required
to sign in at the main office. Parents are requested
to not disturb their children during class time.
Parents and other visitors may NOT enter classrooms
without permission from the main office.
